Overview
You can copy the last chart submitted for a client to save time when documenting services.
To get started, simply open a new chart for a returning client from the appointment or client profile. If the chart template has been completed for the client before, the option to copy from a previous chart will appear.
Select “Copy from previous” to fill the chart with the previous submission’s responses except for the signature and date.
Turning off the ability to copy previous charts
If you’d prefer that staff do not copy previous versions of a chart, you can turn it off.
Head to Manage Business (the gear icon) > Forms & Charts and select the chart template.
In the chart’s settings, toggle off “Copy from previous.” Staff will no longer be able to copy charts that use this template.
Best practices for copying previous charts
If you wish to copy previous charts to create a running log of appointments like for weight loss, hormone therapy, laser or other frequent treatments, we recommend using the long text component to create plenty of space for new entries.
If you change any of a chart’s components, you won’t have the option to copy a previous chart until a chart has been submitted with the new template.

How does the “copy previous chart” functionality work?
Open a new chart
Boulevard looks for the most recent chart submission that used the same chart template for the client. If it finds one, you’ll be asked if you want to copy the previous responses.
If you say yes, Boulevard makes a copy of all of the responses except for the signature and date.
Any fields connected to the client profile (like date of birth or referral source) will be copied over too.
Make any necessary edits and submit the new chart.
If you use the same chart for a variety of services, that chart will be available for all services.
Why am I not seeing the option to copy a previous chart?
Only works on charts (not forms, which are intended to be client-facing)
Chart status must be “completed” (cannot be copied from “in progress” or “needing review”)
Setting must be ON in the chart template’s settings (manage business > forms & charts > [chart name] > settings).
You must have the Advanced Form privilege
If the chart template (components, etc) has been changed since the last submission, we can’t copy the previous submissions. Once a chart has been submitted with the updated template, we can copy the previous submission
What happens to photo uploads and markups when I copy a previous chart?
A copy is made of the images and those copies are saved with today’s date. They will appear in photo gallery with the new date.
I don’t want my staff copying charts-I want staff to manually complete charts each time. How do I turn off the ability to copy previous charts?
Under Manage Business > Forms & Charts, select the chart name
Go to the template’s settings and toggle off “Copy from previous”
Can I decide to copy a previous chart even when I’ve already started filling out a chart?
Yes, you can decide to copy the previous chart at any point before submitting. Any responses that you’ve completed for this visit will be replaced by the previous submission’s responses.
If you dismiss the original “Copy from previous” prompt and change your mind, you can always go to the menu in the top right corner instead.
How does copying a previous chart interact with connected form fields?
Fields that are set to autopopulate will still autopopulate when a previous form is copied.
If you update a field that is connected to a client profile field, you’ll update the client profile when you hit submit.
